Ellenco sends your first
claim form out with your new policy document. They also send you a new
replacement claim form out each time you send in a claim.
For your convenience, if you have misplaced your original claim form,
please print out the two pages of the claim form by clicking on the links
below. Claims cannot be submitted online, as hard copy and original receipts
are required. Please post your completed claim form to Ellenco with relevant
receipts etc attached as per the claim form instructions.
